BACKGROUND QUESTIONS

When you register for the AEPA, you will be asked to answer the following background questions. Providing accurate and complete background information about yourself is important; the information that you provide will be kept confidential. If you register on the Internet or by telephone, you will answer the following questions interactively as you register. If you register by mail, enter your responses to the following questions on the registration form.

Arizona Preparation Institution (#8A on registration form)

Identify the Arizona institution where you completed or will complete your educator preparation program. If you are registering by mail, refer to the "Arizona Preparation Institutions" table that follows for the three-digit code to enter on the registration form. If you attended more than one institution, indicate the institution where you completed your student teaching or practicum.

If you are attending or have attended an out-of-state college or university, leave this question blank and proceed to the next question.

Your test scores will be released to the Arizona institution that you indicate in response to this question. The purpose of providing institutions with test results is to give them an opportunity to review their educator preparation programs in light of examinee performance and to make any modifications or changes to the programs that might be indi­cated as necessary by the test data.

Student Teaching or Practicum Completed (#8B on registration form)

Have you completed your student teaching or practicum requirement? Please answer yes or no according to your status at the time of the test date for which you are registering.

Alternative Pathway to Certification (#8C on registration form)

Did you complete your student teaching through an alternative pathway to certification? Please answer yes or no.

Years of Teaching Experience (#9 on registration form)

Please indicate the number of years of full-time teaching experience you have had. (Count partial years as full years.) You may count teaching in nonpublic schools, but do not count any of the following: student teaching, substitute teaching in an assignment that lasted less than a full year, home instruction, or college teaching experience. Please answer this question according to your status at the time of the test date for which you are registering.
